Il protagonista è Alex Mercer che si risveglia in un ospedale psichiatrico di New York senza ricordare nulla. Alex fugge dall’ospedale e si ritrova in una città in piena lotta tra l’esercito e una nuova razza di mutanti, creati da un virus. Il giocatore a questo punto potrà decidere se combattere e devastare oppure camuffarsi e nascondersi. Alex stesso ha subito mutazioni del suo corpo che gli permettono di avere poteri speciali per attaccare (lame ossee, artigli retraittili, pugni-martello), per difendere (corazza,scudo) e sensoriali (una visione ad esempio gli permetterà di distinguere gli infetti dagli umani). Altri poteri potranno essere assorbiti man mano dai nemici che ucciderà.Oltre alla completa libertà d’azione, il protagonisat, per recuperare la sua memoria, dovrà completare alcune missioni, parte fondamentale del gioco.Ad una buona trama e un gameplay sorprendente si aggiunge un ottimo livello di dettaglio grafico garantito dall’utilizzo del motore Titanium.
